1. Visit https://formed.org/signup
2. Enter/Select St Philip the Apostle in Green Bay from the scroll down menu in the search bar. Then click the blue "NEXT" button.
   

  3.  Create your new account by entering your full name and email address.

       Important: Remember (or write down and save in a safe spot) which email account you used to open your FORMED account. 

       You will need this to reference/ sign back in for each session.

Please note that you will be asked to be signed in to the email account you entered when logging in to use your FORMED account.

2. Entre/ Seleccione St Philip the Apostle in Green Bay, WI del menu que se desplaza hacia abajo en la barra de búsqueda. Luego dele clic al botón azul “NEXT”.

    3.  Para crear su propia cuenta nueva, entre su nombre completo y dirección de correo electrónico.

       Importante: Recuerde (o anote en un lugar seguro) la cuenta de correo electrónico que utilizó para abrir su cuenta de FORMED.  La necesitará como referencia para ingresar a cada sesión.

Tenga en cuenta que se le pedirá estar conectado/a a su cuenta de correo electrónico para ingresar/ usar su cuenta de FORMED.
